Assuring your dignity and helping you find independence is part of our mission. That's why we bring our care to you - wherever you live. For some, it's the house they've lived in for 30 years; for others, it's the home they've made in a long-term care facility. We offer the care you need based on the severity of your illness, your caregiver support system and your living situation.

Our staff live and work near you from seven offices located throughout our 35-county service area. That's another part of our mission: providing community-based care.
