The Hospice Care Team
The Hospice of Central Iowa care team provides expert medical care, pain management, emotional support and spiritual guidance. At the heart of the hospice team are the patient, family caregivers and the patient’s physician. Patients and families choose the services right for them, and our hospice professionals work together to fill those needs. Members of the Hospice of Central Iowa care team include:
- Patients and Families who guide the team’s efforts
- Patient's Physician and/or a Hospice of Central Iowa Medical Director
- Nurses who:
- assist with pain and symptom management
- teach care practices to the patient and family
- Hospice Aides who:
- help the patient bathe, walk and get in and out of bed
- remind the patient to take medications
- change linens and do light housekeeping
- Social Workers who:
- help the patient and family manage ongoing challenges
- provide information about financial assistance, hired caregivers and end-of-life planning
- Spiritual Care Counselors who:
- support patient and caregiver spiritual needs as requested
- respect patient and caregiver spiritual systems
- Pharmacists, Therapists, Dieticians and others who:
- offer special skills, therapies and knowledge to meet the unique needs of the patient and family
- Trained Volunteers who:
- help with errands or prepare light meals
- can assist with personal care or provide relief for caregivers
- provide transportation to medical appointments
- Bereavement Counselors who:
- offer counseling after the death of a loved one
- provide educational materials and learning opportunities
- offer support for up to one year
